1- How much is it for the initial SIM card ?
2- It was my understanding that as soon that we have added credit for more than 100$ (even by processing many credits), then we were qualified "Gold Reward" and starting at that point the card was valid for a year?

1. a blank SIM (no credit, but we will activate the SIM card for you) is US$17
2. Not exactly. Gold reward only apply to pay as you go (pay by minute) accounts. Yes, you refill up to $100, even with multiple refills, you get gold rewards like you said. But if you are on other plans, or you are a gold reward member but changed to other plans, you will NOT qualify for the reward and a refill will only valid for 90 days (unless you refill $100 at once, which will still give you 365 day)
